Application for patient medical records

If you want a copy of your medical records (or if you have written consent from a patient to apply on their behalf), you may apply for this information under the Health Records and Information Privacy Act 2002 (HRIP Act).

Identification documents

Please provide copies of two forms of valid identification which may include drivers licence, Medicare card or passport with your application form.

Payment

Payment of $33.00 (inclusive of G.S.T) is required which can be either done by  credit card or Electronic Funds Transfer (EFT). If you wish to pay by EFT or credit card, please follow the instructions on the application form.

Triple zero call recordings

If you are seeking a triple zero call recording, please do not make an application for patient medical records.
